PATIENT RIGHTS

Every patient of My Left Foot Children’s Therapy, as well as their parents and guardians, have the following rights:

  1. The right to impartial access to medically indicated treatment, regardless of race, color, religion, age, sex, national origin, sources of payment, or diagnosis.

  2. The right to receive considerate and respectful therapy, which is mindful of their spiritual and cultural values and beliefs

  3. The right to consideration of privacy in therapy discussions, consultations, and treatment

  4. The right to expect that all communications and records pertaining to therapy will be treated as confidential, except in cases of suspected abuse or public health hazards, which are required by law to be reported

  5. The right to be cared for by staff who have been educated about patient rights, and their role in supporting these rights

  6. The right to receive therapy in a safe setting

  7. The right to be free from all forms of abuse or harassment

  8. The right to obtain information concerning treatment, in an understandable manner

  9. The right to obtain information about company policies relating to therapy

  10. The right to participate in the development and implementation of an appropriate plan of care

PATIENT RESPONSIBILITIES

Every patient of My Left Foot Children’s Therapy, as well as their parents and guardians, have the following responsibilities:

  1. The responsibility to provide accurate and complete medical information to therapists and staff

  2. The responsibility to follow all clinic policies affecting patient care, conduct, and attendance

  3. The responsibility to treat all staff respectfully and with kindness

  4. The responsibility to be considerate of the rights of all clinic personnel, therapists, and fellow patients

  5. The responsibility to respect all clinic property, as well as the property of other patients

  6. The responsibility to follow the treatment plans and recommendations made by our therapists

  7. The responsibility to ask questions and request additional information if you do not understand any aspects of treatment

  8. The responsibility to be held accountable for all medical, legal, and financial consequences, if you refuse recommended treatment, do not follow the treatment plans, or do not attend scheduled treatment sessions as recommended

  9. The responsibility to provide all necessary, and/or requested information, to ensure the timely processing of insurance claims

  10. The responsibility to handle any concerns or complaints with us directly, by emailing us at: ComplimentsAndConcerns@mlfchildrenstherapy.com. We reserve the right to refuse services (both current services and future services) to people who post negative reviews on social media, such as Yelp, in an attempt to handle a complaint. We will always do our best to make our patients and their families happy if given the opportunity to do so.
